Medicare Advantage Supplemental Benefits

Some Special Needs Plans (SNPs) cover services that support cognitive and emotional wellbeing, especially for individuals with dementia.

Remote Patient Monitoring (RPM)

Remme's cognitive engagement tracking is being developed with CPT codes 99453, 99454, and 99457 in mind, which could support monthly reimbursements of $120–$190 per patient.

Chronic Care Management (CCM)

For individuals with multiple chronic conditions, including dementia, Remme may support care coordination under CPT 99490.

Value-Based Care Alignment

The platform is designed to assist in reducing adverse events (like falls and hospitalizations), helping providers succeed under value-based models.
